The aircraft accident investigation bureau has formed a team to investigate the Ahmedabad plane crash. The crash resulted in 260 deaths. The panel has already submitted a preliminary report. The report indicates a fuel control switch issue and a MAYDAY call shortly after takeoff.

Over the weekend, the Indian Aircraft Accident Investigation Bureau released a preliminary report on last month’s crash of Air India flight 171, which killed 260 people, 19 of them on the ground. The aim of a preliminary report is to present factual information gathered so far and to inform further lines of inquiry.

After India’s crash probe revealed that both fuel switches on the Boeing 787 flipped to “cutoff” shortly after takeoff, raising questions about equipment failure, US FAA and Boeing have assured that the aircraft’s fuel switch locking system is safe and doesn’t require regulatory action.
